The carpet and wall assemblies exhibited intermittent wet and dry conditions, indicating prolonged moisture exposure over an undetermined period. At the time of inspection, the carpet was slightly damp, and mold growth was observed in the ceiling area. No moisture-related issues were identified in the kitchen area behind the dishwasher.

The source of water intrusion was traced to a plumbing leak located between the basement and the kitchen above. The leak originated from a white plumbing pipe with visible black residue on the exterior, indicating prior leakage.

The affected carpet and padding were evaluated and documented as compromised due to prolonged moisture exposure. Wall assemblies were assessed to determine the extent of moisture impact. Repainting was completed on the entire affected wall, from corner to corner, to restore the finished surface following mitigation activities.

Carpet shampooing was performed in areas where visible staining remained. Photographic documentation was completed; however, rust-colored staining was noted as potentially not visible in images and may reappear once the carpet fully dries.

The plumbing source was identified and documented. Plumbing repairs were scheduled, with Dustin assigned to return the following day to complete corrective work on the leaking pipe.

Customer preferences regarding insurance involvement were documented. The customer, Jael/Vic Chris, requested assistance with contacting the insurance provider or participation in a three-way call, as they preferred not to initiate the claim directly. The loss was documented as sudden and accidental in nature.

While onsite, Mr. Vendersliv was contacted, and the completed scope of work was reviewed in detail. All services performed, observations, and conditions were discussed and documented accordingly.
